Henry, do you have to complete this for each individual visit, or can one application cover multiple visits to the US?

It is good for one year! So multiple visits with a year of issuing. Meanwhile, I am still waiting!

It normally takes me about 8weeks if I fax to receive it and it lasts 12 months
